The U.S. Food and Drug Administration has granted approval of Viread, the first nucleotide analogue reverse transcriptase inhibitor for the treatment of human immunodeficiency virus (HIV-1) infection when taken in combination with other antiretroviral agents. The drug works by blocking reverse transcriptase, an enzyme involved in the replication of HIV. As a nucleotide, Viread remains in cells for longer periods of time than many other antiretroviral drugs, allowing for once-daily dosing. This indication is based on analyses of plasma HIV-1 RNA levels and CD4 cell counts in two studies conducted in treatment experienced adults with evidence of viral replication despite ongoing therapy: a 24 week controlled study of Viread and a Viread dose-ranging study of 48 weeks duration.

Viread is an important addition to the antiretroviral armamentarium particularly for therapy-experienced patients who may be resistant to many other antiretroviral agents. Studies in antiretroviral naïve patients are ongoing and will provide information on the risk-benefit ratio in this population.

DOSAGE AND ADMINISTRATION
The recommended dose of Viread is 300mg once daily taken orally with a meal. Since tenofovir is primarily renally eliminated, it is recommended that Viread should not be administered to patients with renal insufficiency (creatinine clearance < 60 ml/min) until additional data become available in this population. The safety and effectiveness of Viread in pediatric patients has not been established.

ADVERSE REACTIONS
The most commonly reported adverse events in patients receiving combination antiretroviral therapy with Viread during clinical trials were mild-to-moderate gastrointestinal events, such as nausea, diarrhea, vomiting and flatulence.

AVAILABILITY
Viread is available as a 300 mg strength tablet packaged in units of 30. It should be stored at 25°C (77°F), excursions permitted to 15-30°C (59-86°F).
